A practical guide
One form, everyone's details.
- Choose one fillable PDF form and one CSV file; the CSV header row must use the form's field names.
- Optionally enter the column used for file names and choose whether to flatten values.
- Fill the forms, then open a few PDFs from the ZIP and compare them with their rows.

Fill a school trip consent form for three children from a CSV with Name, Class and Parent columns.

Before you start: Column names must match field names exactly; unknown columns are reported. Checkboxes accept yes/no, true/false, 1/0 or x. Only standard-font characters are supported.

Fill one PDF form once per CSV row, with columns named after the form fields. Get a ZIP with one filled PDF per row.
Limits and important details
One PDF form up to 10 MB and one CSV up to 2 MB with up to 500 rows. Header names must match field names. Standard-font characters only.

Fill the sample form from its three-row CSV with Reference as the file name column: the ZIP holds A-100.pdf, B-200.pdf and C-300.pdf, each with its own Reference value.
