A practical guide

Fill it in on screen, not on paper.

  1. Choose the PDF to make fillable.
  2. List one field per line: name | type | page | x | y | width | height, in percent of the page from the top left.
  3. Check placement, create the form, then fill every field in a PDF viewer.

Try this example
